Several studies reported that children were the main population at risk for T. canis infection. Currently, no reports on the seroprevalence of T. canis infection in Thailand are available, and its status remains unknown among children who live in rural areas of southern Thailand.ObjectiveThis study aimed to investigate the seroprevalence of T. canis infection and its associated risk factors among primary schoolchildren in rural areas of Nakhon Si Thammarat Province, Thailand.MethodsA total of 177 schoolchildren between 6 and 13 years of age were recruited between June and July 2019. Serum anti-T. canis IgG antibodies were detected with a commercial ELISA kit. A questionnaire administered by direct interviews was used to collect demographic and behavioral risk factor data.ResultsThe overall seroprevalence of T. canis infection was 58.2% (103 of 177). The univariate analysis revealed that schoolchildren who did not practice handwashing before a meal (crude odds ratio (COR) = 3.67, 95% CI: 1.93-6.95, P < 0.001), did not practice hand washing after animal contact (COR = 2.89, 95% CI: 1.53-5.47, P = 0.001), and drank untreated water (COR = 1.87, 95% CI: 1.00-3.48, P = 0.049) had an increased risk of acquiring T. canis infection. However, after adjusting for confounders, only a lack of handwashing before a meal remained a significant risk factor (adjusted odds ratio (AOR) = 2.20, 95% CI: 1.11-4.34, P = 0.023). Age, sex, owning a dog and eating fresh vegetables were not significantly associated with T. canis infection in the current study.ConclusionsThis is the first serological investigation of T. canis infection among schoolchildren in Thailand. The high rate of Toxocara seropositivity reflected high levels of exposure to T. canis among schoolchildren in rural areas of southern Thailand. The results also provide baseline data regarding modifiable risk behaviors for effective T. canis infection prevention strategies in southern Thailand, especially strengthening hand washing practices among schoolchildren.

This research examines geographical accessibility to primary care providers (PCPs) across urban and rural areas of Southwestern Ontario and examines variations in the distribution of PCPs in relation to the senior population (aged 65 years and older). Information about PCP practices was provided by the HealthForceOntario Marketing and Recruitment Agency. Population data were obtained from the 2016 Census of Canada. To calculate scores for accessibility to PCPs (i.e., PCPs/10,000 population), we applied the enhanced 2‐step floating catchment area method with distance decay effect within a global service catchment of 30‐minute drive time. A geospatial mapping approach revealed disparities in the distribution of PCPs with a pattern of higher spatial accessibility in or around major urban areas in Southwestern Ontario. Comparative analyses were performed in association with the seniors’ population to identify how accessibility scores were mismatched with the population needs. The outcome of this study will assist researchers and health service planners to better understand the distribution of existing PCPs to address inequalities, particularly in rural areas.

H. Vu; Paul Dipika;Background Shortages and maldistribution of nurses remain significant problems in many countries. Having appropriate intervention strategies to retain nurses in underserved areas, where they are most needed, are crucial for health system strengthening. This study aimed to quantify attitudes to working in rural areas, perceived competencies, and intention to work among final-year nursing students, and to analyze the associations between those factors and their background characteristics across five countries in the Asia-Pacific Network for Health Professional Education Reforms (ANHER), namely Bangladesh, China, India, Thailand, and Vietnam. Methods A descriptive comparative cross-sectional survey was conducted between July 2012 and July 2013, using a self-administered questionnaire to assess students’ attitudes towards working in rural areas, their perceived competencies, and their intended job choices. A total of 10,169 final-year nursing students in five countries were selected. Bivariate models were constructed to compare students’ characteristics. Statistically significant variables were further analyzed using multivariate models. Results Most nursing students in five countries had rural backgrounds. Students in India (67.1%) and Thailand (65.1%) held more positive attitudes towards working in rural areas. Students in Bangladesh (78.8%) and India (62.6%) believed that their schools prepared them well, and inspired them, to work in rural areas. The ‘Lifelong learning’ competency was ranked highest by students in all five countries, ranging from 76.2 to 91.7%. Their perceived competencies were significantly related to their background of having graduated from rural high schools and being admitted to study through rural recruitment. Rural upbringing and rural recruitment were significantly associated with more positive attitudes towards rural areas (p-value < 0.5). A majority of students in China (83.8%), Thailand (67.7%) and Vietnam (86.5%) intended to work in the public sector immediately after graduation. Conclusions These findings from five Asian countries confirm that nursing students with rural upbringing and recruitment had more positive attitudes toward rural areas and were more likely to choose working in rural areas after graduation. This study provides additional evidence from country implementation to support the value of WHO recommendations of effective strategies to address issues of rural retention by focusing on the recruitment of students with a rural background.

This present study identified the determinants of preventive health check-up utilization among older people in rural Indonesia. This study had been conducted in rural areas in the district of Banyumas, Central Java, Indonesia. A cross-sectional survey had been carried out and a total of 412 older people were selected by using multistage sampling. Logistic regression determined the predictors of preventive health check-up utilization. The majority of the older people were female (75.5%) with low levels of educational attainments (56.3%). Factors including chronic illnesses and uneducated were associated with preventive health check-ups among older people. Older people with chronic illnesses ( p less than 0.05, OR = 6.276) were more likely to use preventive health check-ups. Further, uneducated older people ( p less than 0.05, OR = 12.562) were less likely to use preventive health check-ups in rural areas . Health status becomes a prominent factor that influences health care utilization among older people. Health interventions targeting identified at-risk subgroups are potential for fulfilling the demand for health care among older people who live in the community, particularly in rural areas. Health promotion for enhancing health literacy among older people would improve the utilization of preventive health check-ups among older adults. Abstrak: Pemanfaatan layanan pemeriksaan kesehatan untuk lansia ditentukan oleh kondisi kesehatan dan faktor sosialekonominya. Kajian ini bertujuan untuk mengidentifikasi faktor-faktor determinan pemanfaatan pemeriksaan kesehatan pada lansia di wilayah pedesaan. Kajian ini dilaksanakan di Kabupaten Banyumas, Jawa Tengah, Indonesia. Survei dilakukan dengan menggunakan pendekatan belah lintang dan multistage random sampling terhadap 412 lansia. Data selanjutnya dianalisis dengan menggunakan regresi logistik. Hasil penelitian menunjukkan bahwa sebagian besar (75,5%) lansia adalah perempuan dengan tingkat pendidikan yang rendah (56,3). Status penyakit kronis dan tidak berpendidikan pada lansia berpengaruh signifikan secara statistik dengan pemanfaatan pemeriksaan kesehatan. Lansia yang memiliki penyakit kronis ( p kurang dari 0,05, OR = 6,276) cenderung lebih memanfaatkan layanan pemeriksaan kesehatan, sedangkan lansia yang tidak berpendidikan ( p kurang dari 0,05, OR = 12,562) cenderung kurang memanfaatkan layanan pemeriksaan kesehatan tersebut. KOndisi status kesehatan menjadi faktor utama yang mempengaruhi lansia untuk memanfaatkan layanan pemeriksaan kesehatan. Oleh karena itu intervensi kesehatan yang sesuai untuk kelompok usia berisiko tersebut pontensial untuk dapat memenuhi harapan bagi lansia yang tinggal di daerah pedesaan. Promosi kesehatan untuk meningkatan tingkat literasi terhadap pentingnya layanan kesehatan bagi lansia akan dapat meningkatkan pemanfaatan layanan kesehatan tersebut.

Several studies reported that children were the main population at risk for T. canis infection. Currently, no reports on the seroprevalence of T. canis infection in Thailand are available, and its status remains unknown among children who live in rural areas of southern Thailand.ObjectiveThis study aimed to investigate the seroprevalence of T. canis infection and its associated risk factors among primary schoolchildren in rural areas of Nakhon Si Thammarat Province, Thailand.MethodsA total of 177 schoolchildren between 6 and 13 years of age were recruited between June and July 2019. Serum anti-T. canis IgG antibodies were detected with a commercial ELISA kit. A questionnaire administered by direct interviews was used to collect demographic and behavioral risk factor data.ResultsThe overall seroprevalence of T. canis infection was 58.2% (103 of 177). The univariate analysis revealed that schoolchildren who did not practice handwashing before a meal (crude odds ratio (COR) = 3.67, 95% CI: 1.93-6.95, P < 0.001), did not practice hand washing after animal contact (COR = 2.89, 95% CI: 1.53-5.47, P = 0.001), and drank untreated water (COR = 1.87, 95% CI: 1.00-3.48, P = 0.049) had an increased risk of acquiring T. canis infection. However, after adjusting for confounders, only a lack of handwashing before a meal remained a significant risk factor (adjusted odds ratio (AOR) = 2.20, 95% CI: 1.11-4.34, P = 0.023). Age, sex, owning a dog and eating fresh vegetables were not significantly associated with T. canis infection in the current study.ConclusionsThis is the first serological investigation of T. canis infection among schoolchildren in Thailand. The high rate of Toxocara seropositivity reflected high levels of exposure to T. canis among schoolchildren in rural areas of southern Thailand. The results also provide baseline data regarding modifiable risk behaviors for effective T. canis infection prevention strategies in southern Thailand, especially strengthening hand washing practices among schoolchildren.

This research examines geographical accessibility to primary care providers (PCPs) across urban and rural areas of Southwestern Ontario and examines variations in the distribution of PCPs in relation to the senior population (aged 65 years and older). Information about PCP practices was provided by the HealthForceOntario Marketing and Recruitment Agency. Population data were obtained from the 2016 Census of Canada. To calculate scores for accessibility to PCPs (i.e., PCPs/10,000 population), we applied the enhanced 2‐step floating catchment area method with distance decay effect within a global service catchment of 30‐minute drive time. A geospatial mapping approach revealed disparities in the distribution of PCPs with a pattern of higher spatial accessibility in or around major urban areas in Southwestern Ontario. Comparative analyses were performed in association with the seniors’ population to identify how accessibility scores were mismatched with the population needs. The outcome of this study will assist researchers and health service planners to better understand the distribution of existing PCPs to address inequalities, particularly in rural areas.

Having appropriate intervention strategies to retain nurses in underserved areas, where they are most needed, are crucial for health system strengthening. This study aimed to quantify attitudes to working in rural areas, perceived competencies, and intention to work among final-year nursing students, and to analyze the associations between those factors and their background characteristics across five countries in the Asia-Pacific Network for Health Professional Education Reforms (ANHER), namely Bangladesh, China, India, Thailand, and Vietnam. Methods A descriptive comparative cross-sectional survey was conducted between July 2012 and July 2013, using a self-administered questionnaire to assess students’ attitudes towards working in rural areas, their perceived competencies, and their intended job choices. A total of 10,169 final-year nursing students in five countries were selected. Bivariate models were constructed to compare students’ characteristics. Statistically significant variables were further analyzed using multivariate models. Results Most nursing students in five countries had rural backgrounds. Students in India (67.1%) and Thailand (65.1%) held more positive attitudes towards working in rural areas. Students in Bangladesh (78.8%) and India (62.6%) believed that their schools prepared them well, and inspired them, to work in rural areas. The ‘Lifelong learning’ competency was ranked highest by students in all five countries, ranging from 76.2 to 91.7%. Their perceived competencies were significantly related to their background of having graduated from rural high schools and being admitted to study through rural recruitment. Rural upbringing and rural recruitment were significantly associated with more positive attitudes towards rural areas (p-value < 0.5). A majority of students in China (83.8%), Thailand (67.7%) and Vietnam (86.5%) intended to work in the public sector immediately after graduation. Conclusions These findings from five Asian countries confirm that nursing students with rural upbringing and recruitment had more positive attitudes toward rural areas and were more likely to choose working in rural areas after graduation. This study provides additional evidence from country implementation to support the value of WHO recommendations of effective strategies to address issues of rural retention by focusing on the recruitment of students with a rural background.

This present study identified the determinants of preventive health check-up utilization among older people in rural Indonesia. This study had been conducted in rural areas in the district of Banyumas, Central Java, Indonesia. A cross-sectional survey had been carried out and a total of 412 older people were selected by using multistage sampling. Logistic regression determined the predictors of preventive health check-up utilization. The majority of the older people were female (75.5%) with low levels of educational attainments (56.3%). Factors including chronic illnesses and uneducated were associated with preventive health check-ups among older people. Older people with chronic illnesses ( p less than 0.05, OR = 6.276) were more likely to use preventive health check-ups. Further, uneducated older people ( p less than 0.05, OR = 12.562) were less likely to use preventive health check-ups in rural areas . Health status becomes a prominent factor that influences health care utilization among older people. Health interventions targeting identified at-risk subgroups are potential for fulfilling the demand for health care among older people who live in the community, particularly in rural areas. Health promotion for enhancing health literacy among older people would improve the utilization of preventive health check-ups among older adults. Kajian ini bertujuan untuk mengidentifikasi faktor-faktor determinan pemanfaatan pemeriksaan kesehatan pada lansia di wilayah pedesaan. Kajian ini dilaksanakan di Kabupaten Banyumas, Jawa Tengah, Indonesia. Survei dilakukan dengan menggunakan pendekatan belah lintang dan multistage random sampling terhadap 412 lansia. Data selanjutnya dianalisis dengan menggunakan regresi logistik. Hasil penelitian menunjukkan bahwa sebagian besar (75,5%) lansia adalah perempuan dengan tingkat pendidikan yang rendah (56,3). Status penyakit kronis dan tidak berpendidikan pada lansia berpengaruh signifikan secara statistik dengan pemanfaatan pemeriksaan kesehatan. KOndisi status kesehatan menjadi faktor utama yang mempengaruhi lansia untuk memanfaatkan layanan pemeriksaan kesehatan. Oleh karena itu intervensi kesehatan yang sesuai untuk kelompok usia berisiko tersebut pontensial untuk dapat memenuhi harapan bagi lansia yang tinggal di daerah pedesaan. Promosi kesehatan untuk meningkatan tingkat literasi terhadap pentingnya layanan kesehatan bagi lansia akan dapat meningkatkan pemanfaatan layanan kesehatan tersebut.
